Corporate Director of Home Care

The Corporate Director of Home Care is responsible for developing, administering, and coordinating the business plan and all activities of Home Care operations and services.
This role will be responsible for the initial development of a community based Home Care program, the deployment of the program, ongoing administration and oversite of all Home Care services, and the development and execution of the Home Care strategic growth plan.

EDUCATION AND EXPERIENCE REQUIRED:
Bachelor's degree in Business Administration, Healthcare Administration, or equivalent experience required; equivalent experience equals Five (5) years in a Home Care leadership role Five (5) to Ten (10) years of healthcare leadership or management experience preferred Two (2) to Five (5) years of Home Care operational experience required REQUIRED KNOWLEDGE:
Broad knowledge of home care models and operational models Knowledge of clinical practices and processes, legal and regulatory requirements and mandates Ability to gather and evaluate data and outcome results to use in planning operations, budgets, and process improvement Knowledge of Home Care current and projected healthcare trends Outstanding written and verbal communication skills Exceptional people management, leadership skills, and the capacity to relate to people in a manner that wins confidence and establishes support Ability to position the organization for the future, conceptualize key trends and identify changing market demands, and modify existing strategies to meet the changing demands Strong business acumen and capacity to think strategically and implement tactically Ability to understand major objectives, develop meaningful action steps and facilitate high-level multidisciplinary group decisions Proficient computer skills, particularly with Microsoft Office suite Extensive experience facilitating and managing challenging situations; this includes conflict resolution and demonstration of outstanding interpersonal skills Demonstrated ability to work independently and within cross functional teams.
JOB
Responsibilities:
Assist with the strategic development, and is responsible for the deployment and integration of Home Care services across PSL.
Develop standards and methods to measure quality assurance and performance within Home Care.
This includes driving performance on the identified metrics.
Oversight of and accountability for the performance of all Home Care and Adult Day Program operations Collaborate with PSL clinical operations and related business entities to integrate services with value-based care programs.
Align incentives and accountabilities to key organizational strategies and regulatory standards.
Provide outstanding customer service and accept responsibility in maintaining relationships that are equally respectful to all.
Act as Home Care Services expert for the organization.
Plan overall development and administration of Home Care Services per regulations and established standards.
Develop operational and administrative policies/procedures for Home Care.
Lead the selection, development and deployment of home-based programs and strategies for the overall organization.
Coordinate the annual evaluation of Home Care services; prepares and presents related reports to the quality performance committee (QAPI).
Prepare Home Care reports for the Board of Trustees and related committees.
Support in the development and maintain oversight of the deployment of the Home Care Services strategic growth plan.
Assess, plan, direct, implement and evaluate care and business models and contributes to the overall organization decision-making process.
Oversee the QAPI Programs in each of the agencies and programs; continuously evaluate critical aspects of care and client safety.
Identify breakdowns in customer service and identify solutions to create customer focused systems, processes and procedures.
Manage and develop the administrative service delivery teams to meet all applicable regulatory licensure and accrediting regulations.
Foster effective collaboration between all staff and affiliating services to meet the organization's strategic goals and objectives while ensuring the effective and appropriate use of resources.
Establish and/or maintain working and collaborative relationships with the healthcare provider (pre-acute, acute, and post-acute) community, as well as strategic community healthcare relationships.
Develop and collaborate with Population Health leaders to create care pathways based on analytics and best practice standards and leveraging Home Care Services to bring value to populations served.

Estimated Salary: $20 to $28 per hour based on qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
